We understand that the transition from the educational programming to adult services can be a difficult and stressful process for students and families. In order to facilitate the transition, we work closely with families to prepare them for their direct involvement with government agencies. Each student’s Individual Transition Plan meeting typically coincides with their final IEP meeting as a means of beginning the transfer to adult services. Prior to graduation, we accompany families on visits to ‘After 22 Programs’ to help them explore the options available for their son/daughter. Once the family selects a program, we are available to help them meet that program’s admission requirements (i.e. complete paperwork and organize doctor appointments).
We support our students in this process by introducing them to their new program both in person and through multimedia presentations. These multimedia presentations include pictures and/or video about: how the student communicates; how the student participates in their ADLs; what medical supports are needed and how it’s provided; what type of learning activities they are involved in and motivated by; and behavior plans (when relevant). Adult service providers are invited to our school to observe the student in a familiar and comfortable environment. We also accompany our students to their new program if needed.
